Duties and Responsibilities:
The First Episode Psychosis (FEP) Case Manager will provide comprehensive case management and support for individuals experiencing their first episode of psychosis. This role involves working within a multidisciplinary team to deliver person-centered care, coordinate treatment, and connect clients with community resources, to support their recovery. Conduct thorough assessments of clients presenting with first episode psychosis to determine their needs, strengths and treatment goals. Ensure seamless integration of services and communication between the client, their family, The ideal candidate will have strong communication skills, a passion for helping others, and a commitment to working collaboratively with a dynamic, recovery-oriented team.
Minimum Qualifications:
Bachelor’s degree in social work, psychology, psychosocial rehabilitation or related human service field. A minimum of one-year more years of direct clinical experience withing with children, youth and families supervised experience in mental health. Maintain valid driver’s license. Must have proficient typing and computer skills.
