You will assist clients in obtaining medications and medical care particularly in uninsured clients.  Monitor vital signs for clients and monitors client medication as prescribed by a physician.  Completes a physical assessment for each client within 2 business days or ensures the assessment has been completed within 2 business days.

Bachelor’s degree (B.A./B.S.) / Registered Nurse (R.N.) / Licensed Practical Nurse (LPN) from four year college or university and two to four years related experience.  Must have proficient typing and computer skills in (Word, Excel and Microsoft Outlook)

Outreach services are required to educate and inform each Class Member of their rights under the Williams and/or Colbert Consent Decrees and the opportunities and resources available to them should they explore transition to a Community-Based Setting. Outreach services (of which there is a preference for peer-based provision of individuals with lived recovery experience and/or individuals who have successfully transitioned from a SMHRF or NF to a Community-Based Setting).
