Community Support Team Case Manager

Duties and Responsibilities:

The right candidate will provide case management and counseling for consumers utilizing CST services. As part of the CST Team, you will provide specific, measurable, and individualized mental health rehabilitation interventions to person’s served. Furthermore, you will assist consumers in the development of optimal developmentally appropriate community living skills, and in setting and attaining consumer defined recovery/resiliency goals.

Minimum Qualifications:

Bachelor’s degree (B.A.) from four-year College or university in psychology, counseling, or social work. Meets eligibility requirements as a Mental Health Professional as defined in Medicaid Rule 132. Experience working as part of a CST or ACT team preferred but not required. Must have proficient typing and computer skills. Must possess a valid driver’s license and have a clean driving record.
