The Bridges Program provides intensive community-based mental health services to youth involved with Juvenile Court, Probation Services, or Special Education programs. The Bridges Program provides intensive in-home family therapy and care management services to children, adolescents, and their families. Assessments are conducted at Juvenile Court, in schools, and in the child’s home. The youth and/or his/her family members are linked to needed services and resources to improve their emotional, developmental and behavioral functioning.
Individuals are eligible for services when one or more of the following criteria are met:
Youth is referred for services by Juvenile Court and/or Probation Department
Child attends a Special Education Program (ED/BD) within Bloom, Bremen, Thornton or Rich Townships.
Referrals made by PACE Elementary School and SPEED Therapeutic School will be given priority; and Family is need of intensive in-home family therapy.
