The Child and Adolescent Home-Based Services Program helps youths age 0-17 who are having challenges in the way they are developing socially or emotionally, or are having challenges adjusting to changes at home or at school. The Home-Based Team goes into the home, school or neighborhood to help families in need together.
Primary Services Provided:
Home-Based counseling and psycho-educational services
Intensive Case Management
Psychiatric evaluation and medication monitoring
School-Based mental health services
In home respite services
Individuals are eligible for Home-Based services when any one of the following criteria are met:
History of two or more psychiatric hospitalizations;
History of involvement with multiple service providers, including at least three of the following
Public or private psychiatric hospital;
Public or private residential treatment facility;
Juvenile justice system;
Special Education (BD/ED);
Substance abuse treatment provider;
Comprehensive Community Based Youth Service (CCBYS) program;
Illinois Department of Children and Family Services
History of three unsuccessful outpatient treatment episodes, and youth presently at risk of exclusion from home; or
For in home respite services only, it is sufficient that a youth is registered as a consumer of Grand Prairie Services and at risk of placement outside the home.
